PATIENT RIGHTS & RESPONSIBILITIES

At Pediatric Associates of Stockton, we are committed to helping your child live a healthy life. We are also dedicated to establishing a relationship with you and your child that is based on mutual respect, trust and one that promotes effective health care.

To this end, we have created the following Patient Rights and Responsibilities. These form the foundation upon which our Physicians, Nurse Practitioners, Staff, Parents, and Patients can build such a relationship.

PATIENT RIGHTS

  • You and your child have the right to be treated with courtesy and respect, with appreciation for your dignity and with protection of your right to privacy.

  • You have the right to know who is providing medical services and who is responsible for your child’s care.

  • You have the right to know what patient support services are available.

  • You have the right to know what rules and regulations apply to your conduct.

  • You have the right to be given information by your health care provider concerning diagnosis, planned course of treatment, alternatives, risks, and prognosis.

  • You have the right to refuse any treatment, except as otherwise provided by law.

  • You have the right to impartial access to medical treatment or accommodations, regardless of race, national origin, religion, physical handicap, or source of payment.

  • You have the right to participate in decision making regarding your child's health.

  • You have the right to a prompt and reasonable response to questions and requests.

  • You have the right to treatment for any emergency medical condition that will deteriorate from failure to provide treatment.

  • You have the right to file a grievance in accordance to your insurance company’s grievance procedure as described in your Member Handbook.

PATIENT RESPONSIBILITIES

  • You have the responsibility to provide to your health care provider (to the best of your knowledge) accurate and complete information about present complaints, past illnesses, hospitalizations, medications, and other matters relating to your child's health.

  • You have the responsibility to report unexpected changes in your child’s condition to their health care provider.

  • You have the responsibility to report to your child’s health care provider whether you comprehend a contemplated course of action and what is expected of you.

  • You have the responsibility to keep appointments and, when unable to do so for any reason, notifying the health care provider within 24 hours.

  • You have the responsibility to have knowledge of the result of your actions if you refuse treatment or do not follow the health care provider’s instructions.

  • You have the responsibility to assure that the financial obligations of your child’s health care are fulfilled by prompt payment.

  • You have the responsibility to be familiar with your insurance coverage, using only providers as specified by your plan (except for emergencies inside or outside the coverage area).

  • You have the responsibility to follow Pediatric Associates of Stockton facility rules and regulations affecting patient care and conduct.

  • You have the responsibility to respect the rights of fellow parents and patients.

  • You have the responsibility of carrying your insurance I.D. card with you at all times.

  • You have the responsibility of advising your insurance company, as soon as possible, whenever you receive emergency care, whether inside or outside the service area

ABOUT OUR PICTURES

Our office features a Rain Forest motif that is both fun and educational for our patients. Our lobby, hallways and exam rooms are decorated with murals of jungle animals, each of which is labeled with both common and scientific names. The photos on our website feature many of the same animals that patients will see when they visit our office!
